Heart Failure Accreditation Workshop

This workshop is designed to assist facilities in their preparation for Heart Failure Accreditation. Facilities’ representatives will learn to navigate effectively through the accreditation process by participating in an interactive classroom-style presentation. They will have the opportunity to meet with the Accreditation Review Specialists and learn from the challenges and successes of other facilities that have gone through the accreditation process. Topics will include:
Part I - Accreditation 101 – Overview
Part II - Heart Failure Accreditation – SCPC recognizes the need to provide facilities with an Accreditation Tool to improve their processes and outcomes of the HF patient. Facilities will gain an understanding of process improvement as a continuous learning and building process. Learn how to improve patient care processes that will result in better patient outcomes.
Part III - The Application Process – How to Get Started - Learn step-by-step how to apply for accreditation and complete the application process to ensure there are no delays. Learn about the actual site review – who should participate and what to expect.
Part IV - The Accreditation Tool- Expect to come away with a detailed explanation of each item contained in the Heart Failure Accreditation Tool along with examples of supporting documentation that SCPC's reviewers will use to determine if criteria are met. Attendees will learn how to perform a gap analysis of these criteria to create a road-map that determines what care processes are needed to improve the needs of the heart failure patient and thus lead to better patient outcomes.
Part V – Key Elements – Learn about the ten key areas in which a facility must demonstrate expertise: 1) Emergency Department Integration with the Emergency Medical Services; 2) Emergency Assessment of Patients with Symptoms of Acute Decompensated Heart Failure—Diagnosis; 3) Risk Stratification of the Heart Failure Patient; 4) Treatment for Patients Presenting to the Emergency Department in Heart Failure; 5) Heart Failure Discharge Criteria from the Emergency Department, Observation Stay or Inpatient Stay; 6) Heart Failure Patient Education in the Emergency Department, Observation and Inpatient Unit; 7) Personnel, Competencies and Training; 8) Process Improvement; 9) Organizational Structure and Commitment; and 10) Heart Failure Community Outreach.
Part VI– Shared Practices – Best practices that have led to better patient care processes and better patient outcomes will be discussed, and the attendees will be encouraged to share their own success stories as well.
Part VII– Networking – Continue to learn with your colleagues across the nation, establish new relationships, share your knowledge and examples, and exchange ideas.
Part VIII– Applying Process Improvement Science to Accreditation – Process improvement skills and tools that are essential for a successful accreditation will be discussed. Core competencies needed to complete the process improvement requirements will be presented with clinical examples and hands-on application.

Facilities' representatives will learn to navigate effectively through the accreditation process by participating in an interactive classroom-style presentation. They will have the opportunity to meet with the Accreditation Review Specialists and learn from the challenges and successes of other facilities that have gone through the accreditation process. Topics will include:

Part V - Key Elements - Learn about the eight key areas in which a facility must demonstrate expertise: 1) Emergency Department Integration with the Emergency Medical System; 2) Emergency Assessment of Patients with Symptoms of Possible ACS - Timely Diagnosis and Treatment of ACS; 3) Assessment of Patients with Low Risk for ACS and No Assignable Cause for their Symptoms; 4) Process Improvement; 5) Personnel, Competencies, and Training; 6) Organizational Structure and Commitment; 7) Functional Facility Design; and 8) Community Outreach.
Part VI - Shared Practices - Best practices that have led to better patient care processes and better patient outcomes will be discussed, and the attendees will be encouraged to share their own success stories as well.

This workshop will assist the “novice” and “experienced” coordinator alike acquire and enhance the necessary skills to blossom as a successful coordinator. Topics to be discussed include Process Improvement, Project Management, Team Leadership and the Basics of a Functional Chest Pain Center Team.
Atrial Fibrillation Certification Workshop

Why attend an A-fib Certification workshop?
As with the education workshops offered by the SCPC for CPC and HF accreditations, this workshop is designed to assist facilities in their preparation for A-Fib Certification. Facilities’ representatives will learn to navigate effectively through the process to become a-fib certified by participating in an interactive classroom-style presentation led by SCPC’s Atrial Fibrillation Clinical Specialists. The environment of this workshop will also enable facilities’ representatives to network among each other and share their reasons for pursuing this new certification and their anticipated steps toward success.
What is covered in the workshop?
- The Background and Science of Atrial Fibrillation – Learn the background and current science of a-fib as it relates to assessment, treatment and management. Learn current research findings which include the prevalence and costs of a-fib, risk assessment tools, choice of treatment strategy, new medications, and new oral anticoagulants.
- Atrial Fibrillation Certification – Gain an understanding of the science of process improvement as a continuous learning and building strategy leading to improved patient care and outcomes for a-fib patients. Learn how these process improvement strategies will posture your facility to meet future expectations in healthcare reimbursement.
- The Application Process – How to Get Started – Learn step-by-step how to apply for certification, access the online documents, and complete the application to ensure that there are no delays.
- The Certification Tool – Expect to come away with a detailed explanation of each item contained in the A-fib Certification Tool, along with examples of supporting documentation that reviewers use to determine if criteria are met. Attendees will learn how to perform a gap analysis of these criteria to create a roadmap that determines what care processes need improvement to meet the needs of a-fib patients and thus lead to better outcomes.

STEMI Boot Camp 2012

This workshop is a systematic approach to optimizing local STEMI Systems of Care. Learn how to:
- Significantly improve STEMI recognition-to-reperfusion times at your institution and/or within your local STEMI System of Care by developing a coherent "STEMI Alert process" that includes real-time data collection on every STEMI.
- Insure that your STEMI management and treatment strategies are compatible with currently evolving best practices-based recommendations as promoted by the Society of Chest Pain Centers and the American Heart Association's Mission:Lifeline initiative.
- Develop the education strategies that will allow you to quickly and efficiently train your STEMI providers who are working in the trenches regarding the importance of their role as "STEMI Gatekeepers."
- Learn important strategies for forming cooperative partnerships with surronding institutions and EMS agencies to proivide better regional care for all STEMI patients within your local area.
- Understand the principles of systems engineering as they apply to STEMI Systems of Care - enabling you to reap the most bang for your buck within your efforts at STEMI systems improvment.
Workshop Registration Fees:
Each course is a stand-alone workshop. Pre-Conference Workshops and Boot Camps are not included in the conference registration fees. Workshops are limited in size and pre-registration for these courses is required.
