When an Accreditation Toolkit is purchased, participation for one facility representative at a Chest Pain Center Accreditation Workshop is included; all other facility representatives are $495.00 each.

Acceptable forms of payment include: VISA, MasterCard, Discover, and American Express. Checks and P.O.s will not be accepted.
Confirmation
You will receive an email confirmation once you have completed the registration process. Please bring your confirmation with you to the on-site conference registration desk.
Cancellations/Refunds
Conference cancellation requests are subject to a $50 processing fee from the time of registration until April 21, 2012. Cancellations on or after April 22, 2012 will not receive a refund.
